I find it interesting that only the British journal groups responded, whereas the American did not...I guess the AMA, ACP, and Massachusetts Medical Society have nothing to say?

Yes, potentially a conflict of interest/publication bias...

The reprint costs for the Lancet are staggering (to me, at least).
