Risk Mgmt. is looking for documentation that 10-14% is the average number of self-reported incidents/events, i.e. in organizations that self-report adverse events/incidents, only 10-14% are actually reported.  I have been trying various text word combinations in Google and PubMed but am getting nowhere.  Can anyone suggest a search strategy or source?  Thank you!!
