We went with this:

By definition it is is pretty open and anonymous so it does open itself to crap postings but so far not too many and it has given us some useful information. The backend is a shared spreadsheet - to Administrators and SRO.

I am sure there are much better options but this was a need it quick solution that has remained useful.
