For a right ax-fem-fem: You would then use 93931 and 92926?

Jeff the origin of a fem-fem is typically the femoral artery (hence it's name - Bazinga! - but you're a tough Preds fan so you will get over that jab).
Since the femoral artery is a peripheral artery, it follows that one needs to code its evaluation with 93926 (or -25) in addition to the inflow (93979).
To exclude the 93926 would, in my humble opinion, leave earned money on the table.
If the bypass was from the iliac to the controlateral femoral then I would omit 93926.

A fem to fem is a lower extremity arterial study.
You can bill for the inflow as a 93979 (unilateral/ limited) and then the fem-fem which would be either a 93926 if you only studied the graft and it's outflow or a 93925 if you did a complete bilateral study. And of course ankle waveforms/pressures can also be tacked on (93922).
