I have used LimeSurvey in the past (until Tim Raymond consolidated all of the CEMS evaluations using a homegrown solution).

We use a homegrown online course evaluation written in PHP and backed by MySQL.  Our version asks 3 questions -- "How hard was the course", "How good was the instructor" and "How much did you learn"  (all phrased differently) with the choices being 1 to 5.  We could add/change questions and we can get text answers but we're not set up for conditionals.
