Try the following, as a start:

        Knutsen SF
Life style and the use of health services.
        Am J Clin Nutr 1994 May; 59(5 Suppl):1171S-1175S

Most of the studies I was able to find on this topic in the current
Medline file relate to hospitalization for a specific condition (as in,
lifestyle factors that relate to hospitalization for psychiatric
conditions, or for relapse after treatment for something), but there are
many interesting leads if you combine MeSH for LIFE STYLE and
HOSPITALIZATION or LIFE STYLE and PATIENT ADMISSION, and I assume that at
least some of the bibliographies in these papers must have other relevant
material.
